What is a Statement of Health?

A statement of health is a document that asks you a series of questions about your health.  Oftentimes, completing this form is a requirement for securing life insurance coverage.

When Do I Need to Complete a Statement of Health?

While there are many situations in which you will need to complete a statement of health.  Some of the most common reasons for filling out this form include the following:

  • Requesting coverage that exceeds the maximum guaranteed issue amount on your plan
  • Enrolling after the open enrollment period has ended, maybe because you are adding a dependent
  • Changing your mind after waiving all or part of your employer-paid insurance

How Do I Complete a Statement of Health?

The specifics of your statement of health will differ depending on your group plan.  However, in general, you will have 60 days to complete and return the form.  Many plans offer participants the ability to access and submit statement of health forms via their online portal.

Does a Statement of Health Require Me to See a Doctor?

While statements of health do not typically require a doctor's visit, you may have to submit an Attending Physician's statement or undergo a paramedical exam if you answer "yes" to any of the questions on the statement.  Paramedical exams are physical examinations arranged by your insurer and conducted by a medical professional.  This quick, 30-minute exam will be covered by your insurer.
